EEC Forms & Guidance for Educators




Along with new regulations, EEC has also released forms in order to assist Educators in maintaining licensing standards.

*NEW!*Child Care Enrollment Packet
Describes the information that parents must give to their provider upon enrolling their child in a Family Child Care program such as emergency contact numbers, allergies and other important information about their child.

*NEW!*Parent Handbook
This handbook and enrollment packet, given to parents by Family Child Care providers, outlines many policies and procedures that relate to childcare, including required information that providers must give to parents upon enrollment.

Parental Permission Forms
Contains several sample forms that family child care providers can use when getting parental permission for specified purposes.

Family Child Care Licensing Forms
These forms are used during the licensing process for both new licensees and renewals.

Record Keeping Forms
Use these forms when keeping records about your Family Child Care program. Includes evacuation and smoke detector forms, injury reports, the child care record chart and more.

Making Changes to a Licensed Program
Use these forms when you need or want to make changes to an already licensed Family Child Care program such as change of address, variance request, change in licensed space or capacity and more.

Assistant Documentation Checklist
Complete this form to provide information about any assistants you may have on staff.

Authorization to Release Information (from Previous Employer)
Filling out this form gives the Department of Early Education and Care (EEC) the authorization to obtain any and all information about your employment history.
